Passport Requirements

1. Two 2″x 2″ identical color pictures to be sent in with application
2. A certified copy of your birth certificate to be sent in (will be returned)
3. Present driver’s license to Clerk to be copied front and back — picture ID requirement
4. Make out checks to:
   • Department of State — for $130.00 (per passport)
   • Lindsey Sramek — $35.00 (statutory fee/per passport)
5. Must appear in-person to sign application form in front of Clerk of Court
6. If need to “expedite” the fee is $60.00 more — will get within 3 weeks

For a child under age 16

Both parents must appear and present the following:

1. Evidence of Child’s US Citizenship (birth certificate)
2. Evidence of Child’s relationship to parents (birth certificate)
3. Both parents’ identification (driver’s license — Clerk will copy)
4. Both parents must sign the application
5. Two identical 2″x 2″ color pictures of the child

Cost for child 15 years and younger

(valid for 5 years)

  • Lindsey Sramek — $35.00 (statutory fee)

Other Fees

To renew a passport book — $130.00 (use form DS-82) You mail in.

1. Passport card — age 16 & over (DS-11)
   • Department of State — $30.00
   • Lindsey Sramek — $35.00 (statutory fee)
2. Passport Card — age 15 & under (DS-11)
   • Department of State — $15.00
   • Lindsey Sramek — $35.00 (statutory fee)
3. To renew a passport card (adults) (DS-82)
   • $30.00 (You mail in).

The $35.00 statutory fee is not charged when adults 16 and over, with fully valid passport book, applies for the passport card (use DS-82 form).
